Since it's been long out of print, has it ever been archived in pdf form anywhere?

Yes, or not as a PDF but many many years I go I ruined a copy and scanned it. Here is a download for it

I didn't ruin your copy Draco, I ruined the one I got that was in new condition

EDIT: Hope you enjoy it. And thanks for posting about it, I should do some editing to it and actually make a nice PDF for it.
